Questions regarding insurance

We do not bill or accept insurance.  Upon receipt of payment and completion of the appointment, we will provide a superbill for you to submit to your insurance company for reimbursement consideration.  For coverage, contact your insurance provider and give them the CPT Code 97802GT and 97803GT for out-of-network medical nutritional therapy services.
